The district nursing team provides home and clinic-based health care to the people of the Kaipara region. Nurses cover north to Waipoua Forest Settlement, south to mid-Paparoa township and east to Tangiteroria. The Pouto peninsula is fully covered.
The service includes following up with home health care after a hospital stay, wound management, continence assessments and education, palliative care service on weekends, generalist nursing and support for specialist nurses. Assessments and referrals are made to other health services as required.
Referrals to the service may be made by any health professional from hospitals, hospice or palliative care organisations, iwi providers, and GPs.
