Ward 15 includes a 10-bed Acute Stroke Unit and a 20-bed Assessment & Rehabilitation Unit for people post stroke, surgery, medical event and trauma.
Patients are generally admitted via the inpatient wards and at times as a planned admission from the community for rehabilitation.
The ward is managed by a senior nursing team, led by a Clinical Nurse Manager, who are available Monday to Fridays during the day. The nursing team of Registered Nurses work over three shifts through 24 hours. There is a Nurse Coordinator on an afternoon shift seven days a week and in the weekends on the morning shift. Patients are assigned a nurse each shift. Health Care Assistants support the nursing team.
A Geriatrician or stroke team will manage your health care; the team consists of a House Office, Registrar and Consultant Physician. Occasionally there will be Medical Students as part of the team.
Monday to Friday – Rehab patients are seen by one of the Geriatricians twice a week. If unwell, this is increased.
On weekends and Public Holidays rehab patients are seen as required by the On Call Medical team.
Acute stroke patients are seen daily by the Stroke Physician.
There is a Multi-disciplinary team (MDT) working across the Ward who consists of Ward Clerk, Social Workers, Dietician, Speech Language Therapists, Physiotherapist and physiotherapy assistant, Occupational Therapist and occupational therapist assistant, Therapy Assistants, Psychologist, Pharmacist and Takawaenga (Maori Health Liaison. Chaplains are available on request. Each patient will have a team of appropriate therapists allocated to them.
Families are welcome to support our patients. Kaitiaki (Partners in Care)/ Key support people as nominated by the patient are welcome to be present on the ward outside visiting hours. The Medical team and MDT are available to speak with whanau Monday to Friday 8.00am - 4.30pm. Outside of these hours the nursing will be available and help coordinate times with the Doctors and MDT.
